Referrals

If your Provider refers you to a specialist or for outside testing, that request will be sent to our referral department. The Referral Coordinators will make the necessary appointment(s) and contact you by phone with the date and time of your appointment. A letter of confirmation of the Date and Time of the appointment will also be sent to your address. You should hear from them within 4-5 business days. If you have any questions, call one of our Referral Coordinators at (207)355-3514.

Primary Care Physician

If you have Primecare insurance coverage you must contact them to select your PCP Before your appointment. If this is not done, you may be responsible for the charges due at that appointment. This also applies if your insurance plan is an HMO, such as Anthem

Your Feedback Matters

Do you have a concern, suggestions or comment for us? Press 8 when you call the office and leave a message on our patient feedback line. This is checked daily. Your input is very important to us.

What to expect

Once you arrive , please expect the following:

  1. Go to the front desk and check-in.
  2. Pass in the forms that were mailed to you when you made your first appointment. (If you do not have these ask the receptionist for new forms.)
  3. Show Proof of who you are, where you live, and insurance, if you have it.
  4. Pay Co-pay if you have one.
  5. You will be escorted to an exam room by a medical assistant, then your provider will come in.
  6. Remember to stop at the checkout counter with your paperwork.
  7. Make necessary follow-up appointments, or lab work if needed.

If a revisit is scheduled

Always remember to bring:

  1. Medicare, Mainecare, Health Insurance Card.
  2. List of your prescription medicines.
  3. Any change of address information.

If you are having an emergency, please go to the nearest emergency room on call 9-1-1.
