Referrals

If your Provider refers you to a specialist or for outside testing, that request will be sent to our referral department. The Referral Coordinators will make the necessary appointment(s) and contact you by phone with the date and time of your appointment. A letter of confirmation of the Date and Time of the appointment will also be sent to your address. You should hear from them within 4-5 business days. If you have any questions, call one of our Referral Coordinators at (207)355-3514.

Primary Care Physician

If you have Primecare insurance coverage you must contact them to select your PCP Before your appointment. If this is not done, you may be responsible for the charges due at that appointment. This also applies if your insurance plan is an HMO, such as Anthem

Do you have a concern, suggestions or comment for us? Press 8 when you call the office and leave a message on our patient feedback line. This is checked daily. Your input is very important to us.

Patient Feedback / Complaint Process

The Staff at Sebasticook Family Doctors works hard to offer you quality healthcare and services. If services do not meet your expectations, we welcome your feedback.

To Provide Feedback:

  1. To register a complaint with us you may do so in the following ways:
    1. Call 368-5189 and select option 8
    2. You may voice your complaint to any staff member
    3. You may request a Patient Complaint Form and give this to any staff member.
  2. The individual receiving your voicemail or personal complaint will record it and take it directly to their supervisor.
  3. A Director will contact you within 24 hours to let you know we received your complaint and are taking steps to correct it.
  4. Within 30 business days of the date of your original complaint, a Director will contact you with a solution.
  5. If you are still not satisfied, your feedback will be forwarded to our CEO for final resolution.
  6. If for some reason Sebasticook Family Doctors cannot resolve your grievance to your satisfaction, you have the right to:
    1. Discontinue your care and treatment at Sebasticook Family Doctors.
    2. Transfer to another provide within Sebasticook Family Doctors.
    3. Contact the consumer division at the State Attorney’s General office. 

If you are having an emergency, please go to the nearest emergency room on call 9-1-1.
