If you have Primecare insurance coverage you must contact them to select your PCP Before your appointment. If this is not done, you may be responsible for the charges due at that appointment. This also applies if your insurance plan is an HMO, such as Anthem

Self-payment and co-pays are expected at the time of service. We accept cash, check, debit card and credit card.
